Research summary

SS-31 (elamipretide)(Elamipretide, Bendavia)

Regulatory status
Accelerated FDA approval (Forzinity, Sep 2025) to improve muscle strength in Barth syndrome; other indications remain investigational.
Strongest evidence
Approved for one rare disease; mixed results elsewhere
Overview

Elamipretide is a mitochondria-targeted tetrapeptide that binds cardiolipin in the inner mitochondrial membrane.

What researchers study it for
  • Barth syndrome
  • Primary mitochondrial myopathy
  • Dry age-related macular degeneration (phase 3)
Mechanism

Binds cardiolipin, a lipid essential to the structure of the mitochondrial energy machinery.

Evidence (3)
  1. Approved drug

    First approval (Drugs, 2026)

Granted U.S. accelerated approval in September 2025 for Barth syndrome patients weighing at least 30 kg - the first disease-specific Barth treatment.[1][2]

  2. Human · small / uncontrolled

    TAZPOWER extension (Genet Med 2024)

Open-label extension in 10 Barth patients (8 reached week 168): improvements in walking distance and fatigue scores, with injection-site reactions most common.[3]

  3. Human · randomized trial

    MMPOWER-3 (Neurology 2023)

218 adults with primary mitochondrial myopathy. The trial did not meet its primary endpoints.[4]

Limitations of the evidence
  • Approval is for one ultra-rare disease and is accelerated (confirmatory evidence required).
  • Products on this site are sold for laboratory research only. They are not the medicines used in the trials above, and those results do not apply to research material.
